SEEM Collaborative seeks to fill the position of Physical Therapist to provide in-person services to students in our member districts and our SEEM schools if needed.
Qualifications:
Master’s degree preferred. Must be certified and registered by the National Board for Certification of Physical Therapists. Must have reliable transportation and be able to meet the essential functions of the job.
Scope of duties:
- Delivers physical therapy services in accordance with the student's IEP in either a school or home-based setting.
- Consults with student’s teachers and/or caregivers as deemed necessary by the district-based team.
- Attends IEP meetings and other meetings as deemed necessary by the district-based team.
- Provides written evaluations and reports at least on a quarterly basis and otherwise as required.
- Evaluates students’ physical therapy needs and develop goals and objectives to be considered as part of the IEP process.
Employment is contingent upon passing a SAFIS fingerprint and CORI screening.
